Respiratory problems, especially disorders related to the structure of the jaw and face, can significantly affect the quality of life of people. These problems may be caused by congenital anomalies, injuries or developmental changes in the structure of the jaw and face. In this article, we will examine the role of maxillofacial surgery in correcting respiratory problems and provide comprehensive information in this field.

Respiratory problems related to the structure of the jaw and face

1. Obstructive sleep apnea (OSA)

Obstructive sleep apnea is one of the most common breathing disorders that occurs due to airway obstruction during sleep. This condition can be caused by a small or receding jaw, a large tongue, or an obstruction in the nasal structure.

2. Nasal and oral breathing disorders

Structural problems in the nose, such as deviated nasal septum or small nasal passages, can lead to mouth breathing, which itself brings other problems such as dry mouth and snoring.

3. Abnormalities of the jaw and face

Abnormalities such as a small or receding jaw, deviation of the jaw, or a narrow maxillary arch can interfere with normal or sleep breathing.

What is more about maxillofacial surgery?

How does maxillofacial surgery correct breathing problems?

Maxillofacial surgery can improve airways and resolve breathing problems by changing the structure of bones and soft tissues.

1. Orthognathic surgery

In this method, the upper and lower jaws are moved to increase the airway space. This surgery is often performed for patients with severe sleep apnea.

2. Expansion of the maxillary arch

This method is useful for patients who have a narrow upper jaw and experience problems in breathing through the nose.

3. Correction of nasal septum deviation

This surgery helps to remove nasal obstructions and improves air flow.

Maxillofacial surgery procedures for respiratory problems

1. Initial assessment and diagnosis

In the first step, the doctor examines the condition of the patient’s jaw and face structures using three-dimensional images (CBCT).

2. Planning surgery

Detailed planning includes 3D simulation and surgical path design to ensure the patient’s breathing improves.

3. Main surgery

This stage includes changing the position of the jaw bones, correcting the nasal septum or expanding the upper jaw.

4. Post-surgery care

Observance of care points such as using cold compresses, taking anti-inflammatory drugs and soft diet is necessary for faster recovery.

Benefits of maxillofacial surgery in correcting respiratory problems

  1. Increase the quality of sleep
Surgery can improve sleep apnea and snoring and increase the patient’s sleep quality.
  2. Improve the ability to breathe through the nose
Correcting the structural deviations of the nose and jaw enables normal breathing through the nose.
  3. Improved appearance and performance
In addition to correcting breathing problems, this surgery can make the appearance of the face more symmetrical and beautiful.

Deviation of the jaw is one of the common problems in people, which can be caused by congenital factors, trauma or disorder in the growth of the jaw. This condition may cause chewing, speaking and even cosmetic problems. One of the common questions among patients is whether massage can treat jaw deviation. In this article, we will discuss this issue scientifically.

What is jaw deviation?

Deviation of the jaw is a situation in which the upper or lower jaw is out of its natural position and disrupts the facial proportions. This problem can be mild or severe and may require specialized treatments.

Causes of jaw deviation

  1. Genetic and congenital factors.
  2. Bad habits like mouth breathing or finger sucking.
  3. Trauma or injury to the jaw.
  4. Temporal joint disorders (TMJ).

Can massage cure jaw deviation?

Massage and its effect on jaw joints

Massage is mainly used to reduce muscle tension and pain in the temporal joint area. This method can help improve the following symptoms:

  • Reducing pain caused by muscle tension.
  • Improving the range of motion of the jaw joint.
  • Reducing tension and stress in the jaw muscles.

Limitations of massage in the treatment of jaw deviation

However, massage cannot correct the structural deviation of the jaw. Severe jaw deviations are usually caused by bony abnormalities or deeper joint problems and require specialized treatments such as jaw surgery.

What is a dental implant?

Main components of dental implants

  1. Fixture (titanium base):
This part, like the root of a tooth, is inside the jawbone and fuses with the bone.
  2. Abutment:
It is a piece that connects the top of the fixture and the crown of the tooth to the base.
  3. Dental crown:
The visible part that looks like a natural tooth and is placed on the abutment.

Dental implant procedures

1. Initial consultation and review

  • Assessing the health of your mouth and jawbone.
  • Preparing radiographic images and 3D scans for precise treatment planning.

2. Implantation of implant bases (fixtures)

  • At this stage, the titanium base is placed inside the jawbone during a small surgery.
  • The time it takes for the implant to fuse with the jawbone (healing period) is about 3 to 6 months.

3. Placement of the abutment

  • After the base is welded, the abutment is installed as a connector between the base and the tooth crown.

4. Installing a crown or denture

  • In the final step, an artificial crown, designed to match the color and shape of the natural teeth, is placed on the abutment.

Advantages of dental implants

  1. Natural appearance and beauty of the smile:
Implants look exactly like natural teeth and enhance the beauty of your smile.
  2. High durability:
Dental implants can last a lifetime if properly cared for.
  3. Preventing jaw bone loss:
The implant acts like a tooth root and prevents jawbone loss.
  4. Normal function:
Ability to chew, speak, and smile like natural teeth.
  5. No need to shave adjacent teeth:
Unlike a bridge, an implant does not damage the surrounding healthy teeth.

Dental implant procedures

Dental implants are a precise, multi-step process that aims to replace missing teeth and restore the natural beauty and function of the mouth. In this method, the steps are completed in a complete and standardized manner using advanced techniques tailored to each patient’s needs. Below, we have discussed the main steps of dental implants.

1. Initial consultation and examination

The first step in the implant process is a consultation and a thorough examination of the patient’s oral and dental condition. At this stage, the dental surgeon examines the patient’s overall condition and provides an appropriate treatment plan.

Important steps for this stage:

  • Review of the patient’s general health and medical history.
  • Obtaining 3D radiographs or CBCT images to evaluate the jawbone.
  • An assessment of jaw bone density and volume is performed to ensure the possibility of implant placement.
  • Detailed planning of treatment steps according to each patient’s condition.

What is further study about dental implants?

2. Implant base placement and extraction of non-retainable teeth

At this stage, the titanium base of the implant is placed in the jawbone as the root of the artificial tooth during a minor surgery. This titanium base gradually fuses with the jawbone due to its high compatibility with the body.

Details of this step:

  • Local anesthesia to prevent pain and discomfort.
  • Making a small incision in the gum to access the jawbone.
  • Placing the implant base in the desired location and closing the gum.
  • If necessary, bone grafting for patients who have bone loss.

More information about painless implants

3. Implant healing and healing period

After the titanium abutment is placed, the process of bone fusion with the implant (osseointegration) begins. This stage takes time and usually takes between 3 and 6 months . During this time, the jawbone naturally grows and embraces the implant base.

Important points during the restoration period:

  • Maintain oral hygiene to prevent infection.
  • Avoid applying pressure to the surgical site.
  • Regular visits to the doctor to check the progress of the implant.

4. Dental crown installation

After ensuring that the implant is fully fused to the jawbone, the final step is to place the dental crown (covering). The dental crown is installed on the abutment (the connector between the implant and the crown).

Steps for installing a dental crown:

  • Taking impressions of teeth to design a crown that matches your natural teeth.
  • Placing a temporary dental crown to check the bite condition and its alignment.
  • Installation of a final dental crown that is similar to natural teeth in color, size, and shape.

The end result: a natural smile and excellent function.

After the dental crown is installed, the patient can use the implant just like natural teeth. This procedure not only improves chewing and speech function, but also restores self-confidence and the beauty of the smile.

Benefits of following standard implant procedures

  • Reducing the likelihood of complications and increasing the success of treatment.
  • Complete fusion of the implant with the bone for long-term durability.
  • Natural dental design that harmonizes with the patient’s smile.

Advantages of dental implants

Dental implants are one of the best and most efficient ways to replace missing teeth. In addition to restoring normal oral function, this method provides significant benefits to patients. Below are the most important benefits of dental implants.

1. Natural and beautiful appearance

Dental implants are designed to closely match the shape, color, and size of natural teeth, making your smile look completely natural and beautiful.

Appearance benefits:

  • Perfect harmony with other natural teeth.
  • Increase self-confidence by restoring a beautiful smile.
  • Preventing facial shape changes and sagging lips.

Read more about the role of dental implants in smile reconstruction

2. High durability

With proper care, such as good oral hygiene and regular checkups, dental implants can last a lifetime.

High durability factors:

  • Using body-compatible titanium that fuses with the jawbone.
  • No decay and high resistance to pressure and abrasion.
  • Similar care to natural teeth (brushing and flossing).

3. Prevent jaw bone loss

After a tooth is lost, the jawbone in that area begins to resorb. By mimicking the function of a natural tooth root, a dental implant stimulates the jawbone and prevents it from resorbing.

Benefits for jaw health:

  • Maintaining jaw bone density and structure.
  • Preventing facial shape changes due to bone loss.
  • Strengthening and stabilizing the overall structure of the mouth.

What is further study about dental implants?

4. Excellent performance

Dental implants function similarly to natural teeth and restore chewing and speaking to a completely normal state.

Functional features of the implant:

  • Ability to eat hard, chewy foods without worry.
  • Improve speech clarity and prevent speech problems.
  • More stability and comfort than removable dentures.

5. No need to shave adjacent teeth

Unlike dental bridges, which require cutting away adjacent healthy teeth, dental implants are placed independently in the jawbone.

Advantages:

  • Maintaining the health and strength of adjacent teeth.
  • Prevent damage to the natural structure of the teeth.
  • Independent and permanent treatment for missing teeth.

Why are dental implants the best option?

Dental implants are considered an advanced and effective method of replacing missing teeth. This treatment not only helps restore the function of the teeth, but also improves the aesthetics and overall health of the mouth and face. Below, we will examine the most important reasons why dental implants are superior.

1. Restoring the beauty and functionality of teeth

Dental implants are designed to look and function exactly like natural teeth.

Benefits of restoring teeth functionality:

  • Chewing food: Return of full ability to eat a variety of hard and soft foods.
  • Speaking: Resolving speech problems caused by missing teeth.
  • Natural appearance: The design of the dental crown matches the color and shape of natural teeth, making your smile beautiful and natural.

2. Preventing facial sagging caused by missing teeth

Long-term tooth loss causes jawbone loss. This bone loss can lead to sagging of the face and a change in its appearance. Dental implants act like a natural tooth root and prevent these problems.

Benefits of preserving jawbone:

  • Preventing jaw bone loss and degeneration.
  • Maintaining the natural shape of the face and preventing sagging lips and cheeks.
  • Maintaining the strength and overall structure of the jaw and face.

3. Increase self-confidence with a beautiful and natural smile

A beautiful smile has a great impact on increasing self-confidence and improving social relationships. Dental implants restore your self-confidence by completely restoring your teeth and creating a natural appearance.

Effects of implants on the beauty of a smile:

  • Perfect harmony of the implant with natural teeth.
  • Restoring a beautiful, bright smile.
  • Improves the overall shape of the face and makes it look more attractive.

Why are dental implants the best treatment option?

  1. High durability:
With proper care, dental implants can last a lifetime and do not require frequent replacement.
  2. No damage to adjacent teeth:
Unlike traditional methods such as bridges, when placing implants, adjacent healthy teeth do not need to be shaved.
  3. Functioning like natural teeth:
Dental implants function exactly like natural teeth and solve chewing or speaking problems.
  4. Prevention of jaw and facial problems:
By stimulating the jawbone, the implant prevents its resorption and preserves the natural structure of the face.

2. Single tooth implant

Single tooth implants are suitable for people who have lost only one tooth. In this method, a titanium post is independently placed in the jawbone and a dental crown is installed on top of it.

Advantages of single-tooth implants:

  • No need to shave adjacent healthy teeth (unlike dental bridges).
  • Natural appearance and function similar to natural teeth.
  • Preventing jaw bone loss at the site of the missing tooth.

3. Full jaw implant

Full jaw implants are suitable for patients who have lost all their teeth in one or both jaws. In this method, several implant posts are placed in the jaw and a fixed or removable prosthesis is installed on them.

Types of full jaw implants:

  • Fixed dentures: Teeth that are permanently placed on implants.
  • Removable prosthesis: removable for cleaning and hygiene.

Benefits of full jaw implants:

  • Full restoration of chewing and speaking function.
  • Completely natural appearance similar to real teeth.
  • Preventing jaw bone loss due to long-term tooth loss.

4. Implants with bone grafting

In cases where jaw bone loss has occurred, direct implant placement is not possible. In these cases, bone grafting is performed to rebuild and strengthen the jaw bone to provide the necessary conditions for implant placement.

Treatment steps:

  1. Assessing jawbone density and quality with 3D imaging.
  2. Grafting: Using natural or synthetic materials to rebuild bone.
  3. Implant placement: After the healing period, the implant is placed in the graft site.

Benefits of implants with bone grafting:

  • It is better to provide suitable conditions for implant placement.
  • Prevent further jaw bone loss.
  • Improving bone structure and strength for implant durability.

Written by Dr. Shaghayegh Bozorgzadeh , a maxillofacial surgeon Canada
In one of the most challenging recent treatment cases, the patient presented with severe bone loss of the upper and lower jaw. To restore the normal function of the jaw and teeth, hip bone grafting and simultaneous planting of implants were used. Also, we used PRP (platelet-rich plasma) to accelerate the healing process and strengthen the soft tissue of the gums.

Initial examination and treatment planning

This patient presented to us with severe chewing, speaking, and esthetic problems. Clinical examination and CBCT imaging revealed severe jaw bone loss and the need for extensive reconstruction.
To ensure the success of the surgery and the stabilization of the implants, hip bone grafting was suggested for jaw reconstruction and the use of PRP to strengthen the gum soft tissue.

Surgical procedures

The reconstruction and implantation of the implant was done in several stages:

  1. Pelvic bone removal : The pelvic bone was removed using minimally invasive methods.
  2. Bone grafting to the upper and lower jaw : The bone graft was carefully fixed in the jaw area.
  3. Simultaneous planting of implants : The implants were placed simultaneously in the reconstructed areas.
  4. Using PRP for the soft tissue of the gums : Platelet-rich plasma prepared from the patient’s blood was injected in the gum area to help repair the soft tissue faster and improve the gum attachment.

Benefits of PRP for gum tissue

  • Accelerating the healing process : reducing the recovery time and healing the gums faster.
  • Strengthening the soft tissue : increasing the quality of the connection between the gum and the implant.
  • Reduction of complications after surgery : reduction of inflammation and possibility of infection.
  • More natural gum tissue : creating a natural appearance and better function for the gums.

Advantages of this advanced method

  • Extensive reconstruction of the jaws : suitable for patients with severe bone loss.
  • Reducing the number of treatment sessions : Transplantation and implant implantation in one session.
  • Improving function and beauty : restoring the ability to chew and speak and return to a normal appearance.
  • Use of PRP : Strengthening gums and improving surgical results.

Post-dental implant care for

How much does proper post-implant care affect the success of treatment and the longevity of the implant? Paying attention to oral hygiene and following your doctor’s instructions can prevent possible complications and speed up the healing process. Below, we will mention the most important post-implant care.

1. Maintaining oral hygiene

After implant placement, maintaining good oral hygiene is particularly important to prevent infection and gingivitis.

Important health tips:

  • Regular brushing: Clean your teeth and the area around your implants at least twice a day with a soft toothbrush.
  • Use dental floss specifically for implants: Dental floss helps clean between the teeth and implants and prevents the accumulation of plaque and bacteria.
  • Use mouthwash: Use antibacterial mouthwashes as prescribed by your doctor to reduce inflammation.

2. Regular visits to the dentist

Regular visits and checkups are essential to check the condition of the implants and the health of the gums. During these examinations, the dentist ensures the health of the bone and the level of the implant.

Visit scheduling:

  • Initial checkup: 1 week after surgery to check the initial condition.
  • Periodic checkups: Every 6 months to ensure the health of the implant and surrounding teeth.

3. Avoid chewing very hard foods in the early days

After implant placement, it is important to avoid applying excessive pressure to the treated area. This will prevent damage to the implant base and the healing process.

Nutritional recommendations:

  • Consume soft foods such as soup, yogurt, and puree in the first few weeks.
  • Avoid chewing hard foods such as nuts, ice, or hard foods.
  • Avoid using a straw to drink liquids, as it may cause pressure.

4. Avoid harmful habits

Some bad habits can disrupt the implant healing process and increase the chance of treatment failure.

Important points:

  • Don’t smoke: Smoking and tobacco use slow down bone healing and increase the risk of infection.
  • Avoid clenching your teeth: Excessive pressure on the implant can cause the base to loosen.
  • Do not use the implant teeth to open objects: This may damage the implant structure.

  • If you notice any side effects or allergies, inform your doctor.

6. Surgical area care
